For most insurance types, no — you can get covered with your passport. A NIE (Número de Identificación de Extranjero) is required eventually if you become a Spanish resident, but it's not a blocker for getting insured on day one. I know which insurers are most flexible on documentation for new arrivals.
The European Health Card (EHIC/GHIC) covers emergency treatment in Spanish public hospitals — but not specialists, not private clinics, and it ends once you're a Spanish resident. Spanish public healthcare can have waiting times of months for specialists. Private health insurance from €29/month gives you immediate access to private GPs and specialists, often in English, with no waiting lists.
Yes. EU licences are accepted directly. Non-EU licences (UK, US, Australian, etc.) are valid for up to 6 months after you become resident, after which you need to exchange or re-sit the Spanish test.
